Employed Applicant

  • 2(two) Valid ID’s
  • Latest ITR 2316
  • Certificate of Employment (COE) / Payslip issued for the last 2 (two) months
  • Checking Account

OFW / Seaman Applicant

  • 2(two) Valid ID’s
  • Proof of Remittances
  • Latest Contract from the Employer or Certification of Employment (COE)
  • Bank Statements for the last 4(four) months
  • Checking Account

Self-Employed Applicant

  • 2(two) Valid ID’s
  • Mayor’s Permit
  • DTI Registration
  • Latest ITR 2307/1701
  • Bank Statements for the last 4(four) months
  • Checking Account

Corporation Applicant

  • SEC Registration
  • Articles of Incorporation
  • Latest Income Tax Return (ITR)
  • Latest Audited Financial Statement
  • Bank Statements for the last 4(four) months
  • Secretary's Certificate
  • Checking Account
